Below are links to Word document forms for maintaining a chapter. The New Student or Faculty Form should be completed by each new member, returned to the chapter Faculty Advisor, and the original form submitted to the INFORMS Omega Rho Liaison. Additionally, the Student and/or Faculty Order Form is a summary of all new members and should be submitted with each new member order. Orders for new members may be placed at any time. Some chapters even have inductions twice yearly.
